UB04 Claim Forms
UBO4 claim forms have replaced the UB92
forms and are used by hospitals, clinics, laser surgery centers, and
ambulatory surgery centers to bill medical insurance claims to the
insurance carriers. Generally the reimbursement rate
higher for facility billing is higher than regular physician or
professional billing.

than the other healthcare billing form - the CMS 1500 form.
