Medicare Credentialing
Credentialing with Medicare is required to bill Medicare for
medical services whether you wish to be a participating or a
non-participating provider. The credentialing involves completing an
application which may involve several different forms.
When you
credential to become a Medicare provider you may choose to participate
with Medicare or to not participate with Medicare. We offer more
information on the benefits of participating here.
We offer the service of completing
Medicare applications for you. Here's how it works.
- We'll fax or email you a list of the
required information for the appropriate forms you need completed
- Send us the required info to complete the
application
- We will complete your application and
email or fax it back to you within two business days to be
signed and submitted
Make sure you have the right forms completed and done
correctly by using our professional

Here's how to tell what you
need.
A solo provider practicing by himself (or herself) will need
an 855I and an 588 EFT (electronic funds transfer) form, and a 460.
Our charge to complete these is $125.00.
A group of providers who have a PC or LLC (or
any corporation) will need an 855B and an 855I (for each individual
provider) an EFT, a 460 and an 855R if there are two or more
owners. Our charge to complete these forms is $225. A group
application includes up to four individual providers. There is
a $20 charge for each additional provider over four.
If a provider is the sole owner of a corporation, they can
complete an individual application (CMS 855I) and still report the
corporation and EIN/Tax ID number. They do not need to complete a
group application. They can also have other providers who work for
them and reassign their benefits.
Again, the key here is two or more
owners of a corporation require completion of an 855B
and 855I. Single owner of a corporation needs only
an 855I. Michele will name herself
as the contact person for the application process, will handle any
questions Medicare may have regarding the application and will follow
through to the end.
