| HCFA 1500 Instructions
In April 2007 the older version of the HCFA 1500 (12-90) was officially replaced by the new version CMS 1500 (08-05) to accommodate the use of the newly instituted NPI numbers. Medical insurance claims must now be filed on the newer version CMS 1500 (08-05) forms to be considered for payment. It is important that the HCFA or CMS 1500 is filled out accurately and completely for the doctor to get reimbursement. Any errors or omissions on the claim can cause the claim to be denied. You are much more likely to get paid if your claim is filed correctly the first time than if you have to resubmit the claim. We have seen many medical offices where follow up is never done.
