UBO4
Forms are medical insurance claim forms used by facilities such as
hospitals, inpatient and outpatient clinics and ambulatory surgery centers
to bill insurance companies for services rendered. Read

The
UBO4s are similar to the CMS or HCFA 1500 forms, but used by
hospitals and clinics for medical billing rather than physician or other
provider services. Rev codes are used in place of CPT codes and you
are required to use a "type of bill".
UBO4 forms can be a little intimidating
to complete correctly. We get many questions on our "Ask the Biller" page concerning properly
completing these forms. If the UB04s are not completed
correctly, they likely will be denied by the insurance
company. Lately more and more insurance companies are insisting
that claims for facilities be submitted on the UB04 form in order to be
considered for reimbursement.

The
UB O4 forms come with a number of improvements and enhancements over the
UB92 forms. They are more in line with the 837 format and they allow
for the reporting of the NPI number, or National Provider Identifier
number.
The new UB04
forms were available in the beginning of 2007 but they
were not accepted for billing until March 1, 2007. From
March lst to May 22, 2007 providers were allowed to use either the
old UB92 or the new UB04. Starting May 23, 2007 health plans
accepted the new UB04 forms
and format.
If you are
submitting claims electronically, you should contact your software vendor
now to find out what changes need to be made to your system to allow for
the new format.
If you are submitting
your UB forms on paper, you need to make sure your software is capable of
printing the new UBO4 forms. Unless you recently upgraded your
software, most likely it will need some
changes.
It is important that you
submit your medical claims in the format required and on the correct
forms if you want your services paid for.
